WHO IS EXERCISE REFERRAL FOR?

Suffering from a health condition can make you feel low and restricted. It can change your life and ability to function as you perhaps were used to.

Exercise referral is a type of personal training scheme that is aimed at people who live with chronic health conditions. You may wish to refer yourself directly to S.E.E.D.S or a referral may be made through a health care professional directly e.g. your GP, physiotherapist or nurse practitioner. 

The Exercise Referral scheme, aims to work with clients, to build up their functional ability and enhance their quality of life.

Each program will be tailored to your needs and their will be an ongoing conversation between your trainer and health professional to ensure you are delivered the highest quality service and that your exercise is safe for you.

HOW DOES IT WORK?

​First things first, we need a referral, this can happen one of two ways:

  • the client may approach us directly,

  • GP or medical professional can make a referral for their patient

If the client contacts us first, we must then open up conversation with their medical professional to ensure this is the right thing for them.

Once everything has been cleared and the trainer is fully informed with details of the clients' medical conditions, the screening and programming  can begin. 

There will then be several check-ins with the medical professional throughout the journey and an exit strategy will be implemented towards the end of the set program.
